1. HEALTH ISSUES
Martial Arts Training Involves A High And Intense Level Of Physical Activity. Therefore, You Should Check With Your Doctor Before You Start These Classes To Ensure That You Do Not Have Any Health Problems Or Conditions That May Make These Classes Unsafe For You. If During Any Class, In-Studio or Off-Site, You Experience Any Physical Problem, Including Dizziness Or Shortness Of Breath, You Should Immediately Stop Your Participation In The Class And Seek The Appropriate Medical Attention.
In Checking The Box To This Document, I Acknowledge That I Am In The Proper Physical Condition To Participate In Any Martial Arts And I Further Acknowledge That I Have Inspected The Team Renzo Gracie Facilities Or The Chosen Off-Site Location And Agree That They Are Suitable And Safe For The Martial Arts Classes.

2. TERMS OF MEMBERSHIP/CLIENT
During The Term Of My Membership And/or As A Client Of Team Renzo Gracie Orlando, I Agree To Comply With The Rules Established From Time To Time By Team Renzo Gracie Orlando, And My Instructors To Ensure That The Classes Are Orderly, Safe And Fun For Everyone. I Understand And Agree That Failure To Follow The Rules And Directions Of My Instructors Could Disrupt The Class And Also Could Result In Serious Injury To Other Participants. Therefore, I Agree That At Any Time Any Instructor Or Any Other Member Of Team Renzo Gracie Orlando Staff Has The Right To Immediately, In Their Sole Discretion, Terminate My Membership And/or Ability To Be A Client Of Team Renzo Gracie Orlando, Without Refund Of Pre-paid Membership Fees.
Additionally, I Understand And Agree That A 3% Annual Increase Will Automatically Apply To My Membership Fees Every January 1st.

3. WAIVER – COVID-19 / OTHER COMMUNICABLE DISEASE ACKNOWLEDGEMENT & ASSUMPTION OF RISK AGREEMENT
I Understand And Have Full Knowledge Of The Current Status And Evolving Nature Of The Global Coronavirus Pandemic (Covid-19), Knowingly And Voluntarily Acknowledge That My Attendance And Participation In Class At Team Renzo Gracie Orlando Is Completely Voluntary. I Am Fully And Completely Aware That My Voluntary Participation In Class May Subject Me To Communicable Disease Including But Not Limited To Covid-19, Other Coronaviruses, Seasonal Flus, And Colds And Bacterial Or Viral Diseases Of The Skin. I Am Aware Of The Actual Dangers Involved And Appreciate The Nature And Extent Of The Specific Risks Including But Not Limited To Personal Injuries, Fatigue, Illness, Respiratory Failure, Hospitalization And Associated Required Treatments Such As Medications And Ventilation, And Possible Death. I Voluntarily And Knowingly Assume These Risks And Agree To Subject Myself To These Risks By Participating In Class With Full Knowledge Of The Potential Consequences. I Also Agree To Not Attend Any Class If I Have Any Suspicion That I Have Been Exposed To Covid-19, Am Sick, Or Have Any Communicable Disease Of Any Kind. I Specifically Waive My Legal Rights And Agree Not To File Any Lawsuit Against Team Renzo Gracie Orlando, Its Affiliates, Owners, Instructors, Professors, Or Employees In Any Way Related To The Above-Referenced Diseases/Illnesses.

4. WAIVER – PERSONAL INJURY
I Am Waiving The Right To Bring A Lawsuit, To Recover Compensation, Or To Obtain Any Other Remedy For Any Injury Or The Death, Damage Or Loss Of Property, Or Accident Of Any Kind Arising Out Of My Martial Arts Classes With Team Renzo Gracie Orlando.

5. WAIVER – DEMONSTRATIONS AND TRAVEL
I Understand That As A Member And/or Client Of Team Renzo Gracie Orlando, I May Have The Opportunity To Travel To Competitions, Seminars, Workshops Or Otherwise Demonstrate Their Martial Arts Skill Outside Of Team Renzo Gracie Orlando' Premises. My Waiver As Outlined In Paragraph 4 Above, Shall Also Include Any Personal Injury, Death Or Other Loss That May Occur At Any Point During A Trip Or Event Outside Of Team Renzo Gracie Orlando' Premises, Including, But Not Limited To, All Travel To And From Such Events, And Any Injury Suffered During Any Non-Martial Arts Activities.

6. WAIVER – PHOTO / VIDEO / RECORDING RELEASE
I Give Permission For The Use, Publication And Other Display, Including Advertisements, Of Photos, Video And Other Recordings Taken Of Me While I Am A Member And/or Client Of Team Renzo Gracie Orlando. I Understand That I Will Not Be Compensated In Any Way For Any Photo, Video, Or Other Recording Used, Published Or Displayed By Team Renzo Gracie Orlando, Or Others.

7. WAIVER – RELEASE AND INDEMNITY
I Hereby Agree To Release, Indemnify And Hold Harmless Of Team Renzo Gracie Orlando, Including Its Agents, Owners, Officers, Directors, Shareholders, Volunteers, Employees, Chaperones And All Other Persons Or Entities Acting In Any Capacity On Behalf Of Team Renzo Gracie Orlando, From Any Cause Of Action, Claims Or Demands Whatsoever, However Caused, Including, Without Limiting The Generality Of The Foregoing, Any Claims Which Are Connected With: (I) My Participation In Martial Arts, (Ii) My Use Of Team Renzo Gracie Orlando' Facilities; And (Iii) Use Of Photographs, Videos Or Other Recordings Taken Of Me And Including Any Such Claims Which Allege Negligent Acts Or Omissions By Any Of The Persons Listed In This Paragraph. I Agree That This Waiver, Indemnity And Release Of Liability Is Binding Upon My Heirs, Assigns, Personal Representatives And Estate.

8. WAIVER – PRIVACY – COLLECTION OF PERSONAL INFORMATION
Team Renzo Gracie Orlando Has Collected The Personal Information Listed In This Application For The Following Reasons: I) To Be Able To Contact The Applicants Regarding Team Renzo Gracie Orlando Martial Arts And Other Related Activities; Ii) To Be Able To Collect Payment Of The Membership Fees, Including The Release Of The Information To Professional Payment Services, The Company That Processes The Payment Of The Membership Fees On An Instalment Basis; And Iii) To Try To Prevent Injury To Applicants By Being Aware Of Any Health Conditions That Could Affect Their Participation. By Signing This Application, I Agree To Allow Team Renzo Gracie Orlando To Collect, Use And Disclose My Personal Information For These Purposes.

9. CANCELLATION POLICY
Team Renzo Gracie Orlando Operates On A Month-To-Month Membership Basis With No Long-Term Contracts. To Cancel Your Membership, You Must Submit A Cancellation Request In Writing Via Email At Least 72 Hours Prior To Your Next Billing Date. Verbal Cancellations Or In-Person Requests Will Not Be Accepted.
Absolutely No Refunds Will Be Issued For Any Reason, Including But Not Limited To Unused Classes, Missed Sessions, Or Late Cancellations.

10. SUSPENSION POLICY
Members May Request A Temporary Suspension Of Their Membership For A Maximum Of 90 Consecutive Days. All Suspension Requests Must Be Submitted In Writing Via Email And Must Include A Specific Restart Date. Memberships Cannot Be Paused Indefinitely Or Without A Declared Restart Date.
Please Note That Entry-Level Membership Offers, Discounts, Or Promotions Are Not Applicable If You Choose To Rejoin The Gym At A Later Date. In Such Cases, You Will Be Required To Rejoin At The Full Current Membership Rate In Effect At That Time.
